PREHAB Sports Medicine Services accepts most insurance plans including Medicare. If you don’t see your plan listed, please feel free to call us at (401) 463-3060 to see if your plan is affiliated with one of the many insurance providers we accept.
Physical therapy counts as an out-of-pocket expense towards your insurance plan’s deductible.**

Private Insurance
Before arriving for your first appointment, call your insurance provider to ask about your deductible and co-pay. **
If you have Medicare*** with a supplemental plan, please remember to also call your secondary insurance as copays apply.
Workers Compensation
Please call us at (401) 463-3060 with the following information:
- The name of the insurance carrier
- Your date of injury
- Claims adjuster’s company, name and phone number
- Claim Number
Our workers compensation specialist will work as fast as possible to obtain authorization for your physical therapy to begin.
** Please be advised that as of January 2014 many insurance plans may require patients to meet an out-of-pocket deductible before full policy coverage begins. It is the patient’s responsibility to be familiar with the terms of their particular policy’s rules and regulations. Note, Physical Therapy services may be subject to either a deductible, a copay, co-insurance or specialist rate as determined by your particular plan’s terms. Some plans may have a yearly cap that limits the number of visits they will cover. Please call the number on the back of your card prior to your first visit to see what benefits you are eligible for under the terms of your health insurance plan.
Although RI recognizes direct access allowing patient self referral to PT, some insurance plans, including Medicare, do not and may require a pre-authorization and or a referral/prescription from either a primary care physician, nurse practitioner, PA-C or a specialist before starting treatment; at present Medicare does not recognize referrals to PT issued by a chiropractor.
***Medicare allocates a yearly budget of $1900.00 towards Physical Therapy and Speech therapy services combined. If you have had PT, speech therapy, or a combination of the two services since January 1st of this calendar year you may have a greatly reduced or depleted benefit.
If you have Medicare and are transitioning from post-surgical home care to outpatient physical therapy, please be sure your visiting provider has formally discharged you from their services prior to your first appointment date.
